Abstract Granulomatous appendicitis is a rare form of due to variety causes such foreign body, fungi, parasites, mycobacteria, Yesinia, Crohn's pathology and delayed appendicectomy. Definitive diagnosis necessitates histopathological analysis. We present an unusual case granulomatous in young gentleman, incidentally detected at histopathology, along with brief literature review. Patient presented classical clinical history examination findings suggestive acute underwent emergency laparoscopic appendicectomy, followed by uneventful post-operative recovery. Histopathological showed features on chronic transmural inflammation epithelioid granuloma, confirming appendicitis. hereby, aim emphasize the significance increased awareness required among general surgeons pathologists, as patients needs be investigated further likelihood them having underlying Crohn’s disease or developing it future.
